Presidents Message

It is spring and that means the Gerry Haggerty Annual Leadership Institute (GHALI) is right around the corner. For those of you who have not attended this regional conference in the last few years, it is 2 ½ days packed full of the best education and networking $345 can buy. In a time when many organizations are reducing expenses and cutting back on travel and education, this conference really is a great value as it includes all of the following:

  • 20 hours CPE featuring nationally-known speakers and topics that will keep you on the cutting edge of the profession
  • All meals included
  • Open bar each evening
  • Your choice of golf, spa or wine tasting networking events

It’s hard to believe how fast my year as chapter president has flown by. The theme this year has been “Making Connections”, and we are fortunate to have such a strong HFMA chapter to help us stay connected.  We’ve had many accomplishments as a chapter over the past year, including receiving national awards for membership retention and certification. It takes many enthusiastic chapter volunteers to keep our chapter going strong, and I thank everyone for their help and appreciate all of the personal support from the leadership team.
 
Speaking of making connections, did you know that Northeast Ohio HFMA now has a group on Facebook and LinkedIn? If you use these networking websites, look for us and join. This is a fresh, new way for HFMA to offer professional networking, and we have some good ideas as to how to use these resources to benefit members. By the way, you have to be a member to join, so make sure you renew your membership :  )

Patient Financial Services: February 19, 2009

With the expertise of our speaker lineup and key topics of industry interest, this year’s Patient Financial Services meeting was one of the most successful and well attended events yet.  So, what did you miss?

Are You Ready to RAC and/or Roll Over?
Presented by Lyman Sornberger – Executive Director, Cleveland Clinic Health System Patient Financial Services
Cheryl Arnold – Senior Director, Business Development & Training
Rebecca Stewart, CCS, CPC – Project Manager, Business Development

Lyman Sornberger and his team from Cleveland Clinic provided insight into the Medicare RAC process based on their first-hand experience in Florida.  While their Florida experience may have been “out of tune”, the lessons learned have made significant impact on their strategic planning and the efforts put forth for preparing for the Ohio rollout. Specifically, Lyman’s presentation addressed what to expect, what to avoid, and strategies for how to best prepare for and minimize the negative impacts of RAC.    

SornbergerIn a brief review of RAC and its purpose, Mr. Sornberger summarized the RAC claims review process and key risk areas associated with the audits.  Whether the audit is identified via an Automated Review (claim determination at the system level without human review of the medical record) or a Complex Review (those requiring human review of the medical record) – Lyman made a clear point that “once the State comes in to your facility and starts the education, the letters and remits start too”. While several revisions to the rules and guidelines have been published since the demonstration project (i.e. medical record request limits), case in point – you need to be prepared. 

Rebecca StewartIn the latter half of the presentation, Rebecca Stewart discussed more specifically the targeted areas of the demonstration project based on their Florida experience. The more in-depth examples covered included:

• One-Day Stays
• Improper DRGs (those most commonly identified
by the RAC)
• Inpatient Procedures (unrelated procedures or those on     the Inpatient-only list)
• Incorrect Discharge Status (an Automated Review audit)
• Three-Day LOS Transferred to a SNF
• Inpatient Rehab (and criteria for meeting medical necessity)
• Incorrect Units Billed (Neulasta, Outpatient Speech Therapy, Blood Transfusions)
• E & M Services

By learning through experience, Cleveland Clinic has dedicated significant time and effort to the preparation of the RAC rollout, comprised mainly of four key areas:

  • Development of a RAC Steering Committee
  • Risk Assessments
  • Development of Processes to Manage the RAC Rollout
  • Development of a RAC Tracking Tool

Cheryl Arnold talking RACCheryl Arnold walked the audience through their RAC “Working Teams” and the steering committee resources dedicated to completing risk assessments, the logistics and operating model for RAC, education and training, process improvements, and external relations (communicating lessons learned and collaborating on opportunities). Shared in this part of the presentation was a tracking tool created by Cleveland Clinic to determine audit probability (unlikely, likely, very likely) based on the key risk areas known to be targeted by RAC. With this tracking tool, Cleveland Clinic has been able to estimate the resources needed to prepare for and manage the RAC process as well as monitor outcomes, provide trend reporting, and help to identify process improvement areas. As stated by Mr. Sornberger himself – it is better to “fight the fight together than to go at it alone” – so, shared with the audience was a copy of the presentation as well as an open forum for preparing for RAC and ideas/support for managing the process.

Kenneth MorganBuilding Frontline Teams of Excellence –
Patient Focused Staff Collection Competence

Presented by Kenneth W. Morgan, Senior Consultant – Gustafson & Associates, Inc.

“People are the only element with the inherent power to generate value. All other variables – cash, materials, plant and equipment – offer nothing but inert potentials. By their nature, they add nothing, and they cannot add anything until some human being – be it the lowest-level laborer, the most ingenious professional or the loftiest executive – leverages that potential by putting it into play.”*

With this leading quote, Mr. Morgan led the audience through understanding and acknowledging today’s current challenges presented by the revenue cycle process and people models. From Pre-Access, to Time-of-Service, to Post-Service – the current revenue cycle process model remains broken, and as such is a costly organizational model. Once this is acknowledged, Mr. Morgan presented the need to then identify the technical skills and emotional competencies required of today’s “Gold Standard” revenue cycle team members – his “Upside-Down” model – which is patient-preference driven, quality controlled, and aligns to mutual goals, accountabilities, and rewards.

Emotional Intelligence competencies being most essential to this “new” process and stemming superior customer service (skills and communication) are “intrapersonal” – self-awareness, self-regulation, and motivation – and “interpersonal” – effective communication and coaching of others. Mr. Morgan also reviewed with the audience the importance of completing skills assessments to ensure specific abilities and talents required for workflows are defined, leading to more effective and successful processes and employee education and development programs. Provided in Mr. Morgan’s attached presentation is an example of a “Skills Identification” assessment tool which can be utilized as a guide for this key process.

In building a stronger frontline team and one which is patient-focused and point-of-service collection focused as well – Mr. Morgan presented four related “customer service wisdoms”:

  • Customer satisfaction drives healthcare organizations’ long-term success
  • Patient billing-related interactions are a major source of dissatisfaction
  • Non-clinical interactions have as much impact on satisfaction as clinical encounters
  • Patient-friendly processes increase satisfaction and thereby increase long-term organizational success

With these ‘wisdoms’, it is important to identify the purpose and appropriate implementation of staff “scripting” and “role-play” to facilitate patient-orientation and point-of-service collection. Essential communication and scripting elements to the financial counseling process include:  stating the reason for the conversation, informing and educating the patient, clearly stating what is required for the visit/service, achieving a resolution, and finally thanking the patient and answering any questions. Mr. Morgan provided several useful scripting examples to the audience which can also be viewed in his attached presentation copy.

Today’s healthcare providers must ensure financial strength and stability, maintain compliance and customer allegiance despite the challenges related to people and process. With the tools provided by Mr. Morgan, revenue cycle leadership teams will be on the right path to more focused collection success!

Job Search Survival 2009

Undoubtedly, this is the toughest year on record to land a new job. Reaching your career goal will take courage and nerves of steel. Are you up to the challenge? Here are three tips for job-search endurance that will keep you on the right track toward your employment goal.

 1. Keep your career goal realistic.

This is not the time to strike out in a risky career direction. Following your heart toward a career in which you have little qualifications could yield months of frustration as you find yourself competing against legions of candidates far more qualified. Unless you are in the position to hold out for a very long job search, concentrate on positions where you are best qualified.

 2. Realize it will take longer to land your next position.

If you've never experienced a lengthy job search, set your expectations out several months and practice patience. You will apply for many positions as the perfect candidate, and get no response. Expect that. You will conduct perfect interviews and hear nothing back. Expect that as well. Just remember that eventually the right company with the right job at the right time will come your way if you stay calm and focused and don't let discouragement keep you from moving forward. Just keep with it.

3. Write a better resume than your competition.

Less jobs and more applicants equals extremely high competition. The quality of your resume has never been more important. For the best possible resume keep these guidelines in mind:

  • Focus your resume. Avoid a one-size-fits-all resume.
  • Showcase your best information in the top half of page one.
  • Include accomplishments that illustrate your ability to solve today's business challenges.

4. Sharpen your interview skills.

With employers interviewing only the best of the best, when you are chosen to interview be sure you are your competitive best. You CANNOT “just wing” an interview and expect to be called back for a second. Today it takes solid interview strategy to earn a second round of interviews. Interview books are helpful, but they usually fall short of teaching you how to read the interviewer's mind to understand his/her hiring motivations. A study in the art of selling is more effective to achieve great interview performance. A few basic selling strategies include:

  • Asking the right questions to understand the interviewer's “hot button” motivations.
  • Formulate answers around the interviewer's motivations.
  • Know your accomplishments well enough to weave them effectively through your interview to achieve top candidate status.

Throughout 2009, the best jobs will go to those who persevere and stay focused. Keeping your expectations and goals realistic will help prevent the emotional ups and downs. Prepare for your job search as if you were competing in a marathon. With patience, endurance and skill you will win your next job.

HFMA NEO Chapter

Certification Materials and Exam and
Reimbursement Policy & Procedures

Policy

The North East Ohio Chapter (NEOC) of the Healthcare Financial Management Association (HFMA) encourages members to pursue and achieve certification as a Certified Healthcare Financial Professional (CHFP), which includes successfully completing the Core exam and one of four Specialty Exams. To assist members with qualifying for this designation, the Chapter, through the Chair of the Certification Committee, makes self study materials available to members in preparation for both the Core exam as well as the Specialty exam of their choice. To further encourage members to seek certification, the Chapter, through the Chair of the Certification Committee, may reimburse members for CHFP examination fees upon successful completion of each exam and may further reimburse members for CHFP certification fees upon confirmation of certification, provided that these fees are not otherwise reimbursable by the members employer or other source.
 
In order to encourage members to focus their study efforts and to expand the availability of study materials to a greater number of members, self study materials may be borrowed for up to 4 weeks and must be returned promptly to the Certification Committee Chair. Request for reimbursement must be submitted by the member within 8 weeks of receiving their exam results or certification confirmation.

Members who borrow the CHFP study materials from the Chapter are NOT eligible to earn CPE Credit with those materials as HFMA rules limit the number of CPE submissions per set of materials to one. Consequently, NEO Chapter members who wish to receive CPE credits for undertaking one or more of the CHFP examination self study programs are required to purchase their own self study materials. The cost of CHFP self study materials purchased by members will not be reimbursed by the NEO HFMA chapter.

Procedures

  1. Members considering sitting for the CHFP core or specialty exams are encouraged to contact the Certification Committee Chair to arrange to borrow the HFMA self study examination related materials.  The Chair will advise the member if the requested materials are immediately available, or when they are due to be returned. The Chair will sign out these materials to the member one exam at a time for up to 4 weeks.  Members who have already scheduled their exam sitting will be given priority. Members are obligated to return exam materials promptly at the end of four weeks or at the time of examination, which ever is the earlier. Contact information for the Certification Committee Chair is available on the North East Ohio Chapter Website or in the HFMA Annual Handbook. 
  2. Upon successful completion of an exam, a member for whom the examination fee will NOT be reimbursed by their employer or other source may complete and submit an NEO HFMA expense reimbursement form. A copy of the exam result e-mail notification and a release authorizing HFMA National to confirm that the member has passed the exam is required. Application for reimbursement must be made within 8 weeks of receiving exam results.
  3. Upon confirmation that a member’s application for certification as a Certified Healthcare Financial Practitioner (CHFP) has been successful, a member for whom the certification application fee will not be reimbursed by their employer or other source may complete and submit an NEO HFMA expense reimbursement form. A copy of the certification confirmation and a release authorizing HFMA National to confirm that the member has been certified is required. Application for reimbursement must be made within 8 weeks.
  4. The examination self study material lending program is effective January 1, 2009.  The examination and Certification fee reimbursement program is also effective January 1, 2009. Additionally, members who successfully completed either or both CHFP exams in 2008, or, were certified during 2008, or both, and who have not been reimbursed by their employer or other source for examination fees or application fees may complete and submit an NEO HFMA expense reimbursement form before May 1, 2009.
  5. The NEO HFMA Chapter will not reimburse members for the purchase of CHFP self study materials.

NEO HFMA University Program
Encourages Student Membership

Recently NEO HFMA has been selectively visiting local College and University campuses to share the advantages of a student membership with HFMA.  The University Program is designed around volunteered participation from any current HFMA member interested in providing healthcare career insight or presenting a current healthcare affairs discussion to a local student population. 

Brian Esser, an MBA student at Case Western, and one of our newest Student Members, wrote the following about his student membership with NEO HFMA:

 "I was really excited when I discovered HFMA, it seems like an outstanding organization and is exactly the kind of network I have been looking to be a member of. I think it will be invaluable not only to develop my skills as a financial professional, but really allow me to focus on healthcare, which is where I want to take my career. HFMA has also become a critical tool in my job search that I am using to supplement my academic experience in learning the language and depth of healthcare related matters in finance."

The Membership committee is excited about the potential of actively engaging our local student populations through a NEO HFMA student membership, and welcomes any current member participation. Congratulations to UCB Inc. on celebrating its 50th anniversary supplying account receivable management solutions.
